To view the status of your background check, go to My Profile: Background Check. The following page opens.
Click the View Details button for an explanation of your status.
If you have not completed a form required for any of the checks, the Complete Form button appears in the column on the right. Click this button to open the form.
After you complete the form, you can submit it electronically or print it. Check with your administrator on the preferred way of submitting the form.
